Coordinated Services

Assessment

We provide a professional assessment and plan of action to assist in making the right choices with the aged or disabled individual. The process begins with a face-to-face evaluation of the elderly or disabled individual to determine such areas of concern as functional and cognitive ability, psychosocial resources, family involvement, and financial resources. A plan evolves from this information, and the client is linked to needed services in the community. We have extensive knowledge about the costs, quality, and availability of resources in our community. One call to us will connect you to the appropriate services, saving you time and emotionally costly mistakes.

Coordination of Services

After the Initial Consultation is completed, we implement the plan by coordinating the care and services needed by our clients. This may include placement of caregivers, referrals to other professionals (attorneys, therapists, or physicians), or community services. We monitor the care that our clients receive to ensure quality care and recommend any changes that are in the best interest of our clients.

Crisis Intervention

GeriatriCare® Management, Inc. offers counseling and consultation services for families and individuals experiencing situational problems or crises. A licensed clinical social worker with extensive post-master's degree experience is available and offers ongoing support services to the clients and the family. We will help to resolve the crisis by whatever services are required, including counseling, coordination of caregivers, referral to other professionals, or emergency placement in the appropriate facility. We understand and can assist with many of the common problems faced by the elderly, disabled, and their families, such as grief and loss issues and conflict around care issues.

Noticing a Change of Condition

During our regular visits, we assess our clients and look for any change in status.  By making periodic visits, we are quick to notice a change of physical or cognitive condition.  If there is a concern, we report it to the family and/or physician and determine the best plan of action to resolve the issue.  Whether it’s in a client’s home, in an assisted living facility, or a skilled rehab/nursing facility, we are always checking in to ensure that our clients are well cared for and receive the best quality of life.

Community Resources

With over 35 years of experience in the Washington DC area, we are very knowledgeable about the available resources that meet the specific needs of our clients.  Based on our significant experience, we have come to know which companies are most reputable.  Our clients always receive a personalized resource list to assist with their needs.  Depending on the needs of our clients, the resources that are often needed include Elder Care Attorneys, Home Care Agencies, Durable Medical Equipment Companies, professionals that make home visits and available technology.

Housing and Relocation

We have extensive knowledge of available housing options in the Washington, DC, area, including independent living, assisted living, nursing care, and group homes. While assessing housing options, we make recommendations on placement based on our client's medical needs and financial situation. If necessary, we will locate facilities and apply for an Auxiliary Grant or Medicaid.

Our network of professional relationships throughout Northern Virginia with experts in the field of dementia care includes geriatricians, neurologists, psychiatrists, neuropsychologists, and many others who are devoted to helping ensure those living with memory loss are able to maintain their independence and dignity. We can help you evaluate treatment options and provide family members with the tools and support they need to help maintain a meaningful relationship with their loved one.If an individual with cognitive impairment wants to remain in their own home, we'll make recommendations for in-home care as well as modifications that could help the individual live more safely. If this is not a viable option, we can help you find alternative living solutions utilizing our extensive knowledge of, and experience working with, care facilities throughout Northern Virginia. Unlike placement services or agencies, we accept no referrals fees from facilities to recommend them. We recommend solely on the basis of our experiences and knowledge of quality care metrics.
